NorthLakes Community Clinic is offering a preventative school-based dental program to all Chequamegon School District students in grades 4YK-12. This program is funded in part by Wisconsin Seal-A-Smile in collaboration with Children's Health Alliance of Wisconsin and the Division of Public Oral Health Program. A dental professional will come to the school to provide dental services at NO CHARGE to families. The treatment which your child will receive in this program is not meant to be an alternative to regular dental care. It is still strongly recommended that you continue routine care with your family dentist. If you do not have a family dentist for routine dental care, including any follow-up care which may be recommended after your child has completed this school based oral health program, NorthLakes is committed to helping you find one for your child.
Due to the late start in the 2021-2022 school year, this program will include:
• Oral health education and dental supplies
• Oral health assessment by a licensed dental professional
• Fluoride varnish application
If you would like to have your child participate in this program, you will receive a report describing your child's dental health status and what was completed at each visit. This report will be shared with your dentist and the school nurse. All procedures will follow recommendations from the American Dental Association and Center for Disease Control and Prevention for school-based dental programs. NorthLakes is committed to following these recommendations as they change with the pandemic.
You may sign up online at https://sealasmile.wisconsin.gov/Consent/Home/About. Once the form is completed, it will automatically be sent to the NorthLakes Superior Smiles Coordinator.
If you have any questions, please feel free to contact Kristiana Karshna, the Chequamegon School District Nurse, at (715) 762-2474 ext. 2278 or email@example.com.
